Player Profile FormsThe information provided on the Player Profile Form will be used to create the Player Profile Books provided to College Coaches attending the 2008 Champions Cup. The Player Profile Forms should only be completed for U16, U17 and U18 teams. Obviously, it’s important that this information is accurate, and must be provided to the Tournament Director no later than March 28, 2008. The files for both the Blank Player Profile Form and the Sample Player Profile Form are Microsoft Excel files. The Blank Player Profile Form is for you to complete, and the Sample Player Profile Form is provided only to illustrate how it should appear when you are finished.

Save the file(s) to your computer. Once you have done this, enter the information as shown on the Sample Player Profile Form. Please do not vary from the format that is provided. Do not change type styles, type sizes, column widths, column heights or any other part of the form’s configuration. Please note that all the players should be listed in numerical order by jersey number, not alphabetically. When you have completed the form, email it as an attachment to the Tournament Director, TBD mbarry@thundersc.org.

All teams are required to check-in at the Marriot Courtyard - Dublin (
MAP
) between 6:00 PM and 9:00 PM on Friday, April 18, 2008.

The following items will be required for you to check-in:
(1) 2 copies of your Official Team Roster (kept)
(2) Players and Coaches Passes (laminated with picture attached - returned)
(3) Original Permission to Travel Form (if applicable - kept)
(4) Notarized Club Medical Release Forms for Players (returned)
(5) Coach’s/Team Administrator’s Information (hotel and cell phone numbers)
